WHAT’S NOT INCLUDED IN YOUR MEMBERSHIP

If you need bloodwork or imaging, we’ll provide an order to complete it at an outside facility. You may use your insurance for these outside services. We’re happy to guide you through the process.

Please note: specialist visits, urgent care, and hospital services are not included in your membership. Our goal is to keep you healthy and reduce the need for outside care.

FACTS ABOUT MEMBERSHIP AND INSURANCE

Direct primary care is not health insurance. We recommend that patients maintain some form of insurance (or a health-sharing plan) to help cover emergencies, hospitalizations, specialists, or services outside of our clinic. Your membership ensures you get accessible, comprehensive primary care; your insurance provides financial protection for the unexpected.

Flat Monthly Fee: Patients pay a predictable monthly membership covering all primary care services we offer.

No Insurance Billing: DPC membership fees are not billed to insurance.

Insurance Still Works: You can use insurance for services outside DPC, such as specialists, hospital care, labs, or imaging.

ENROLLMENT FEE WAIVED FOR ALL
NEW MEMBERS UNTIL JANUARY 1, 2026

MEDICARE PATIENTS

We cannot enroll patients who have any form of Medicare at this time. We anticipate being able to accept Medicare patients by January 1, 2026. We will be keeping a wait list of interested patients until that date and these patients will be guaranteed a spot in our practice with no enrollment fees should they choose to join our practice.

CANCELLATION POLICY

We understand that circumstances change, and you may need to cancel your Direct Primary Care membership.

Cancellation: You may cancel your membership at any time with written notice. Your membership would complete at the end of the monthly billing cycle - we do not prorate the last month of membership.

Re-Enrollment: If you decide to return after cancelling, a $250 re-enrollment fee will apply and we cannot guarantee re-enrollment.

*Price only available with an enrolled adult.

A one time enrollment fee of $100 will be added to the first month’s bill.

Family max $250/month (2 adults in same household, up to 3 children under 21). Additional children billed at $25/month

EMPLOYER DPC BENEFIT

Some employers are now offering Direct Primary Care (DPC) memberships as part of their benefits package. This means you may be able to access personalized, comprehensive primary care at no additional cost or at a reduced monthly fee through your workplace. Check with your HR or benefits team to see if your company offers a DPC membership—you might already have access!
